CMS Broadens ACCESS Model to New Chronic Conditions

CMS announced expansion of its outcomes-based ACCESS model beginning in spring 2027 to cover heart failure, COPD, substance use disorders and tobacco cessation, making roughly three in four Medicare beneficiaries eligible.
